WHAT IS THE DIFFERENCE BETWEEN AN OBSERVERSHIP AND AN EXTERNSHIP? Observership is an unofficial position at a hospital wherein you basically just observe, attend rounds or conference but have no real responsibilities and are not allowed to participate in procedures or patient physical examinations. Externship is a quasi official position wherein you are allowed a certain degree of patient contact albeit with close supervision. Most often in observership you "volunteer" , in externship you PAY to get US clinical exposure (although oftentimes it varies from hospital to hospital) . Internship in the USA (or PGY-1 Residency Year) is the first year required year towards securing a US Medical license.

Hospitals in California usually ask for an additional requirement called the California Evaluation Letter.
